Double-Struck with a 5-degree rotation between strikes. The doubling is visible on the central design, lettering, and date. Also, the coin is a partial collar strike, and there is a 5% indent on the reverse at 1 O'Clock.
Buffalo nickel double-strikes are very rare, and most examples are in lower VG or Fine grades, unlike this PCGS MS-64 example.
