NGC 2000-D Jefferson nickel struck on 2000-D Lincoln cent double-denomination, and also double-struck, with the 2nd strike 85% off-center. The double-denomination shows the designs of both the nickel and the cent, as well as both "2000-D" dates. 

The coin is also double-struck, with the off-center second strike. Rare, and with very few known!
