PCGS 1965 Washington quarter struck on a clad dime planchet and with a huge 75% partial brockage. Dramatic wrong planchet error and brockage combo!
The error occurred when a dime planchet was fed into a press that was striking quarters, and was struck with a Washington quarter laying on top of the dime planchet. The result is a coin with a huge brockage on the obverse, and also a coin with the design of a quarter but the planchet of a dime.
