NGC (1999-P) Jefferson nickel double-denomination struck on a Roosevelt dime, with the coin having the designs of both a nickel and a dime.

The designs of both the nickel and dime are strongly visible. The date on the nickel is not visible, and the coin is dated based on the 1999-P Roosevelt dime date which can be seen. Although it's possible that the nickel would be a different date, it's unlikely. 

Double denomination nickels on dimes are rare, with only about a dozen examples known. 

MS63 with problem free, mostly white surfaces.
